Episode One opened with meeting Park Shi Ohn (Joo Won), our hero, who lives alone. He's autistic by highly functional and is leaving to move to Seoul for a residency.
We get a flashback showing how hard life was for him as a child.
The other kids beat him up because he was different. His father was an abusive alcoholic, who in a fit of rage killed Shi Ohn's pet bunny. Shi Ohn had one champion: his older brother, Yi Ohn, who defended him against the other kids and his father.While on the train to Seoul, Shi Ohn interacts with a little boy, Hyun Woo, who is traveling with his mother.
He also was friends with a kindly doctor, Choi Woo Suk (Chun Ho Jin), who acted as a surrogate father and encouraged Shi Ohn's interest in becoming a doctor.
Yi Ohn tried to help Shi Ohn fit in, and in trying to get the other boys to play with Shi Ohn, agreed to go into an abandoned mine to get a trophy. But once the boys entered the mine, the mine collapsed, trapping them inside.
Woo Suk is able to find them, lying unconscious, but the unstable mine collapses, trapping him as well. The accident sadly kills Yi Ohn.
After the train arrives at the station, Shi Ohn is distracted from his journey by a 3-D television. He's amazed at what he sees and tries to touch the floating cartoons.
A cable holding up a huge sign breaks, shattering its glass casing. Hyun Woo happened to be under it and ends up badly bleeding and impaled with glass.
Another doctor starts to treat Hyun Woo, but Shi Ohn stops him before he kills the boy. Shi Ohn steps in and begins to treat his patient. Each step of his process is narrated with a voice over and often Hanguel is supplied to define the medical terms that are being used.
Shi Ohn is forced to jerry-rig objects for use. Hyun Woo is badly hurt and Shi Ohn shows a lot of skill in treating the boy to save his life. The crowd that surrounds them watch intently and many capture the action with their camera phones.
Even after the EMTs arrive, Shi Ohn is the most capable and the EMTs act as his assistants, supplying him with the tools he needs to save Shi Ohn. When he announces that Hyun Woo will be okay, the crowd applauds his efforts. He wants to leave to go to his appointment, but gives into the mother's pleas and rides with them to the hospital. Unfortunately, he left his bags and his phone at the train station.
While in the ambulance, he notes some other symptoms of Hyun Woo, indicating that the boy has some heart trauma that is not showing up on the machines. Once at the hospital, which turns out to be the hospital he was supposed to be going to, the resident on duty ignores his request for a echo-cardiogram and leaves Shi Ohn in the waiting room.
We meet our heroine, Cha Yoon Seo (Moon Chae Won), a doctor who is very good both with medicine and with the children she treats. She gets into an argument with her boss, Kim Do Han (Joo Sang Wook) over the treatment of one of their patients. He's concerned with the girl's physical condition but Yoon Seo wants him to also consider her mental/emotional state.
Do Han's cool demeanor could be influenced by the dead boy whose urn he visited earlier that day. He dismisses Yoon Seo's attitude as that of a kindergarten teacher rather than a doctor. Frustrated, she leaves work early. Do Han ends up being the surgeon who will operate on Hyun Woo.
We also meet the hospital chairman, Lee Yeo Won (Na Young Hee), and her efficiency expert, Yoo Chae Kyung (Kim Min Seo). Chae Kyung is all about the hospital as a business by Yeo Won sees it only as a place with the people being the most important.
Shi Ohn does not remain in the lobby and desperately tries to get Do Han and the other surgeons to do an echo. He is ignored and thrown out of the O.R. But Shi Ohn cannot leave without knowing what happened to Hyun Woo.
Do Han compliments the work of the doctor at the train station, since it made his job much easier and it wasn't something people would normally know how to do. Things are going smoothly, but then there is a mysterious complication. Do Han remembers Shi Ohn's protests and orders the echo, which reveals the hidden trauma.
While Shi Ohn paces outside the O.R., the meeting he was supposed to be attending with the hospital's administrators is happening. They are debating whether or not to accept Shi Ohn as a resident. Woo Suk is arguing that Shi Ohn is more than capable, describing his top-notch education and internship. But many administrators, particularly the head of the hospital's foundation, Hyuk Pil, are opposed to Shi Ohn because of his autism. During the course of the surgery, without being able to meet Shi Ohn, the group votes almost unanimously not to take Shi Ohn as a resident. Yeo Won does not agree with this decision.
Outside the O.R., to cope with the stress, Shi Ohn imagines himself performing the surgery. We are treated to simultaneous shots of Shi Ohn and Do Han operating, both equally as competent. Hyun Woo is saved.
Do Han reports Hyun Woo's status to the mother and Shi Ohn leaves as soon as he hears that Hyun Woo is okay. He even thanks Hyun Woo for surviving. Do Han wants to meet the doctor who did his first aid and is surprised to hear that he works at the hospital.
Shi Ohn finally arrives at the administrators' meeting. They are long gone. Woo Suk is upset and yells at Shi Ohn for being late, but as soon as he sees Shi Ohn's reaction, he softens and begins to be very loving towards him. Woo Suk wonders why Shi Ohn was late and where the blood on his shirt came from. Shi Ohn tells him that he helped a boy who got hurt at the train station.
Woo Suk takes Shi Ohn outside and gives him the bad news. Both men are sad. Woo Suk takes Shi Ohn out to dinner. While at the restaurant, a news report featuring Shi Ohn's work at the train station is aired. Woo Suk is shocked and amazed. Shi Ohn doesn't seem to care what's said; he's concentrating on the food. The uploaded video has gotten the nation's attention, and when reporters show up at the hospital wanting to interview Shi Ohn, the board reconsiders their decision.
At the new meeting, the main opponents of hiring Shi Ohn are still arguing against it. But the notoriety from his work at the train station is working in Shi Ohn's favor and Yeo Won is actively lobbying for his hire. When Hyuk Pil and others still won't budge, Woo Suk goes as far as staking his job as hospital chief on Shi Ohn's success; if Shi Ohn fails, Woo Suk will step down.
Shi Ohn goes before the board and Yeo Won asks him why he wants to become a doctor. Shi Ohn tells them that he watched his bunny and his brother die and couldn't do anything about it. He wants to become a doctor so that next time, he can do something. And he wants to make money so that he can give a 3-D television to the children in the orphanage where he grew up. Satisfied, Yeo Won announces that Shi Ohn is hired.
After the meeting Yeo Won scolds Woo Suk for staking his job. Woo Suk doesn't care, as long as Shi Ohn gets a chance. Any way, he'll still be a doctor. Elsewhere, Hyuk Pil, Hyun Tae (the deputy chair) and Choong Man (the chief of Pediatric Surgery) are plotting on how they will use the situation to destroy Woo Suk. While Hyuk Pil and Choong Man are enjoying to prospect, Hyun Tae seems strangely detached from events.
Choong Man presents Woo Suk and Shi Ohn to Do Han and the other residents. They're shocked that the new guy is the person who interrupted their surgery earlier. Woo Suk asks them and especially Do Han to take care of Shi Ohn and then leaves. When Woo Suk is gone, Choong Man starts yelling at Shi Ohn but Shi Ohn isn't phased by it.
Do Han goes to Woo Suk and argues with him about Shi Ohn. He does not like his mentor's decision. But because Woo Suk asks him, he decides to try to work with Shi Ohn.
When Do Han interviews Shi Ohn, he's irritated by Shi Ohn's attitude and demeanor. But he continues to be impressed by Shi Ohn's medical ability, even though he doesn't acknowledge it. When Do Han briefly leaves Shi Ohn alone in his office, Shi Ohn is able to solve the dodecahedron Rubik's puzzle that has been stumping Do Han for weeks.
Choong Man is continued to be revealed as selfish and arrogant. He dismisses a patient's pain and the mother's worry and belittles the nurse who bothered him and kept him from going home. He orders the nurse not to bother him at the seminar he'll be attending the next day. The nurse is extremely frustrated but has to apologize anyway.
Yoon Seo has gone to a hospital-themed bar to get drunk. Apparently she's a regular since the bartender knows her. Do Han calls her and yells at her some more before abruptly hanging up the phone. This makes Yoon Seo madder and she ends up *really* drunk.
Do Han goes home and Chae Kyung is there wearing a bathrobe. She tells him about the administrators' meeting and about what Woo Suk did. Do Han is not happy.
Shi Ohn has gone home and is celebrating his birthday with a pyramid of chococakes. A flashback reveals the cookies are just like the ones that he ate with his brother, who gave him a kid's medical kid that included the green scalpel that Shi Ohn is often seen holding and looking at. Hyung also told him that he was extremely smart and not to give up on his dream.
Shi Ohn blows out his candle and a drunk Yoon Seo enters his apartment and begins to strip down to her slip. She doesn't notice Shi Ohn and just climbs into his bed. He tries to get her attention but she's too drunk so he leaves her there, asleep in her bed, staring at her.
The next morning, Yoon Seo screams. She's awoken to Shi Ohn in just his boxers, brushing his teeth and staring at her.
